Housing Outreach Specialist

About the position

The Housing Outreach Specialist is responsible for developing and implementing the health plans housing strategy and programs. This includes the approach to assisting individuals in programs in securing accessible, affordable housing through Federal and local programs. This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands-on engagement.

Responsibilities

• Liaise between housing agencies and the health plan and partner with these agencies to develop and access affordable housing services for members.
• Partner with the CHOICES and ECF lead trainers to develop and implement training specific to affordable housing services in assigned state.
• Support Care Coordinators and Support Coordinators in identifying housing options for members.

Requirements

• Requires a minimum of 3 years experience in developing housing and/or housing policy at a local, state or federal level for special populations such as the elderly, people with disability and/or people with mental health challenges to secure accessible, affordable housing through Federal and local programs; or any combination of education and experience which would provide an equivalent background.

Nice-to-haves

• A bachelor's degree in Social Services or related field highly preferred.
• Experience in Behavioral Health preferred.
• Substance Abuse/Peer Support preferred.
• Bilingual Spanish preferred.
