Location : Village of River Oaks, an Independent Living and Assisted Living with Memory Care senior community Shift Available : Sat - Sun 7a.m. - 3p.m. Must be able to work weekends and holidays.

Requirements

  • Minimum of one year working in acute care or long-term care.
  • Valid Texas LVN license.
  • Current CPR certification.
  • Proficiency in computer skills with EHR experience.
  • Excellent communication and customer service skills.
  • Strong problem-solving skills, good judgment, and attention to detail.
  • Understanding of HIPAA requirements.

Responsibilities

  • Administer medications to Assisted Living and Memory Care residents, ensuring compliance with Federal & State regulations and RCM guidelines.
  • Coordinate care with physicians, other medical professionals, and families regarding residents’ medications, treatments, and needs.
  • Help residents with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, positioning, transferring, mobility, and incontinence care.
  • Ensure timely ordering of all required medications and frequently audit medication carts and narcotics.
  • Handle the admissions of new residents, including all assessments and documentation.
  • Complete evaluations and assist directors with service plans as needed.
  • Respond quickly to all emergencies, maintaining calm and effective communication.
  • Maintain close communication with all departments and staff members to ensure coordinated care.
  • Monitor Medication Assistants' and Care Assistants' documentation for accuracy, timeliness, and completeness.
  • Ensure evaluations and service plans are current and accurate, notifying the Clinical Supervisor of any discrepancies.
  • Collect data for all incident reports and input them as they occur.
  • Manage and facilitate the orientation process for all new employees on your shift.
  • Ensure effective communication between direct care staff and residents’ families.
  • Assist with staff scheduling in collaboration with the Clinical Supervisor as needed.
  • Attend all required in-service training sessions.
  • Maintain a professional appearance and good personal hygiene per company policies.
  • Perform other responsibilities as assigned with a positive and proactive attitude.
