About the position
Theoria Medical is leading the charge in healthcare innovation, offering a unique blend of medical excellence and technological advancement, primarily serving the post-acute and primary care sectors. Our extensive network includes multispecialty physician services and RPM, covering skilled nursing facilities across the country. We are looking for nurse practitioners and physicians eager to drive change and deliver superior care. This position is full-time and exempt, with a focus on providing high-quality, patient-centered, comprehensive primary care services to patients residing in skilled nursing facilities (SNFs). The provider will be responsible for rounding daily, prescribing medications, and managing clinical care, while ensuring timely documentation of all patient encounters.
Responsibilities
- Rounding and managing the clinical care of the population at the assigned SNF.
- Prescribing medications and treatments for residents residing at SNFs.
- Providing high quality, patient-centered, comprehensive primary care services.
- Timely documentation of all patient care/encounters.
- Available for phone/secure messaging consultations with facility nursing staff.
- Completing documentation in Theoria’s proprietary electronic health record (EHR) system, ChartEasy™.
- Being on-site on time for scheduled shifts.
- Addressing acute/emergent issues that arise during rounding time.
Requirements
- Medical licensure in the relevant state.
- Experience in primary care or related field.
- Ability to perform essential functions satisfactorily, with or without reasonable accommodation.
- Reliable transportation.
Nice-to-haves
- Experience with PointClickCare, MatrixCare, or American HealthTech EHR systems.
- Previous experience in skilled nursing facilities.
