About the position
Theoria Medical is seeking a dedicated provider to manage clinical care at skilled nursing facilities (SNFs). This full-time position offers a unique opportunity to deliver high-quality, patient-centered primary care services. The provider will be responsible for daily rounding, prescribing medications, and ensuring timely documentation of patient encounters. The role emphasizes meaningful connections with patients and staff, supported by a network of physicians and providers. The position requires reliable transportation and punctuality for scheduled shifts, which are Monday to Friday from 7 AM to 1 PM local time.
Responsibilities
- Rounding and managing clinical care of the population at assigned SNF.
- Prescribing medications and treatments for residents at SNFs.
- Providing high quality, patient-centered, comprehensive primary care services.
- Timely documentation of all patient care encounters.
- Available for phone/secure messaging consultations with facility nursing staff.
- Completing documentation in Theoria’s proprietary EHR system, ChartEasy™.
- Being on-site and on time for scheduled shifts.
Requirements
- Valid medical licensure.
- Experience in primary care or related field.
- Ability to perform essential functions satisfactorily.
- Reliable transportation.
Nice-to-haves
- Experience with EHR systems such as PointClickCare, MatrixCare, or American HealthTech.
- Previous experience in skilled nursing facilities.
