About the position
Theoria Medical is leading the charge in healthcare innovation, offering a unique blend of medical excellence and technological advancement, primarily serving the post-acute and primary care sectors. Our extensive network includes multispecialty physician services and RPM, covering skilled nursing facilities across the country. We are scouting for the brightest nurse practitioners and physicians eager to drive change and deliver superior care. This part-time position involves managing clinical care at assigned skilled nursing facilities (SNFs) with a focus on patient-centered, comprehensive primary care services.
Responsibilities
- Rounding and managing the clinical care of the population at the assigned SNF.
- Prescribing medications and treatments for residents residing at SNFs.
- Providing high quality, patient-centered, comprehensive primary care services to patients residing in SNFs.
- Timely documentation of all patient care/encounters.
- Available for phone/secure messaging consultations with facility nursing staff.
- Completing provider documentation in Theoria’s proprietary electronic health record (EHR) system, ChartEasy™.
- Being on-site on time for scheduled shifts.
- Addressing acute/emergent issues that arise during rounding time.
Requirements
- Valid medical licensure.
- Experience in providing primary care services.
- Ability to complete documentation in electronic health record systems.
- Reliable transportation.
Nice-to-haves
- Experience with PointClickCare, MatrixCare, and American HealthTech EHR systems.
- Previous experience in skilled nursing facilities.
