About the position
Theoria Medical is leading the charge in healthcare innovation, offering a unique blend of medical excellence and technological advancement, primarily serving the post-acute and primary care sectors. Our extensive network includes multispecialty physician services and RPM, covering skilled nursing facilities across the country. We are looking for nurse practitioners and physicians eager to drive change and deliver superior care. This position is full-time and exempt, with a salary of up to $165,000 annually, and is eligible for a sign-on bonus. The role requires in-person attendance at skilled nursing facilities (SNFs) and involves managing clinical care, prescribing medications, and providing comprehensive primary care services to patients. The provider will also be responsible for timely documentation and communication with nursing staff.
Responsibilities
- Rounding and managing the clinical care of the population at the assigned SNF.
- Rounding daily and prescribing medications and treatments for residents at SNFs.
- Providing high quality, patient-centered, comprehensive primary care services to patients residing in SNFs.
- Timely documentation of all patient care/encounters.
- Available for phone/secure messaging consultations with facility nursing staff.
- Completing provider documentation in Theoria’s proprietary electronic health record (EHR) system, ChartEasy™.
- Being on-site and on time for scheduled shifts.
- Addressing acute/emergent issues that arise during rounding time.
Requirements
- Must be a licensed nurse practitioner or physician.
- Reliable transportation required.
- Ability to perform essential functions of the position satisfactorily, with or without reasonable accommodation.
