About the position

The Rehab Admission & Marketing Representative is responsible for providing case management services to all patients referred to WakeMed Rehab as well as marketing activities to monitor and develop relationships with all referral sources. This includes conducting a thorough clinical evaluation, assessing available resources and facilitating the development of a discharge/transfer plan from the referral location to the appropriate program within WakeMed Rehab system, or recommendations for alternative care options. This position is responsible for managing the planning, development and maintenance of an efficient admission system, establishing a community referral system and maintaining positive public relations with all internal and external contacts. Functioning as an advocate for the patient, works to identify initial goals for rehab and outcome achievement potential and is responsible for determining how available resources can be used and making referrals to appropriate community, financial, support and advocacy resources, verifying available healthcare benefits and is the primary point of contact for all insurance company case managers, providing communication as required for precertification of patients' hospitalization and treatment to all involved parties to facilitate continuity of care for each patient and ensures a smooth transition to the rehab program.

Requirements

  • Licensed Occupational Therapist Required
  • Licensed Physical Therapist Required
  • Licensed Speech and Language Pathologist Required
  • Certificate of Clinical Competence in Speech-Language Pathology Required
  • Licensed Clinical Social Worker Required
  • Maintain Vehicle Operator Insurability Required
  • Bachelor's Degree in Physical Therapy, Occupational Therapy, or Speech Language Pathology Required
  • Master's Degree in Social Work Required
  • 2 Years Clinical Experience in Rehabilitation, Acute Care, or Ambulatory Care Required
