About the position

The RN Wound Care Specialist is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The RN Wound Care Specialist is responsible for assessing, planning and managing wound care for patients in their homes. They will work collaboratively with other healthcare professionals to develop and implement effective wound care treatment plans, ensuring optimal patient outcomes and promoting overall well-being.

Responsibilities

  • Performs comprehensive wound assessments, including evaluation of wound type, stage, and severity.
  • Develops and implements individualized wound care plans based on assessment findings and patient needs.
  • Supports virtually wound care treatments and therapies in accordance with established protocols and best practices.
  • Monitors and documents wound progress, including any changes in condition, response to treatment, and patient-reported outcomes.
  • Educates clinicians, patients and families on wound care techniques, infection prevention, and self-care strategies.
  • Collaborates with physicians, physical therapists, and other healthcare professionals to coordinate and optimize patient care.
  • Maintains accurate and up-to-date patient records, including wound assessments, treatment plans, and progress notes.
  • Ensures compliance with home health care regulations, infection control standards, and safety protocols.
  • Provides emotional support and reassurance to patients and their families throughout the wound healing process.
  • Supports appropriate wound classification and ICD10 code alignment.
  • Reviews Wound Assessment Tool Deviation reports and provide guidance as indicated.
  • Performs other duties as assigned.

Requirements

  • Registered Nurse (RN) with a valid nursing license. Compact license preferred.
  • Minimum of five (5) years of experience in wound care, preferably in a home health setting.
  • Strong knowledge of wound assessment, treatment, and management techniques.

Nice-to-haves

  • Certification in Wound Care (e.g. Wound Care Certification (WCC), Certified Wound Care Nurse (CWCN), Certified Wound and Ostomy Nurse (CWON), Certified Wound, Ostomy, and Continence Nurse (CWOCN)) is required at hire, or to be completed within 6 months of starting position.
