About the position

Bartlett Regional Hospital is searching for a Social Work Case Manager with a focus on Emergency Department Services to join our Case Management team. This position provides social work and discharge planning in predictable situations using established policies, procedures, ACMA guidelines, and NASW code of ethics. The role is divided into three levels: SWCM I, SWCM II, and SWCM III, each with increasing responsibilities and requirements. SWCM I is an entry-level position requiring direct supervision, while SWCM II involves more complex case management with intermittent supervision. SWCM III is an advanced level position that requires a Master's degree and allows for independent planning and implementation of social work care. The position may involve participation in hospital committees and teams as appropriate.

Responsibilities

  • Provide social work and discharge planning in predictable situations.
  • Participate in hospital committees and teams as appropriate.
  • Plan and implement complex individualized social work care.
  • Manage routine and emergency care.
  • Assume basic leadership responsibilities and mentor SWCM I's.
  • Conduct clinical evaluations and assessments.
  • Provide behavioral health assessments and intervention services.

Requirements

  • SWCM I: Three years of postsecondary education within a social work program; must graduate from an accredited social work program within one year of hire.
  • SWCM II: Graduate of an accredited social work program; one year of experience in healthcare, behavioral health, or substance abuse treatment.
  • SWCM III: Master's Degree in Social Work preferred; two years of experience in healthcare, behavioral health, or substance abuse treatment.
  • TEAM/BERT Certification within 90 days of hire.
  • Alaska Social Work License within one year of graduation for SWCM I; at time of appointment or within 90 days for SWCM II; required for SWCM III.
  • Valid AK Driver's license required if assigned to perform patient transport duties.

Nice-to-haves

  • Accredited Case Manager certification.

  • Pay rate determined based on applicable personnel rule or union contract terms for internal hires.
  • Hourly rates: SWCM I starts at $34.93, SWCM II at $38.69, SWCM III at $41.60.
