Transitional Care Liaison- Sales and Marketing

About the position

Touchstone Communities is seeking a dynamic Transitional Care Liaison to drive census growth through strategic sales and referral development in the Killeen/Harker Heights region. As a Liaison, you'll represent Touchstone's skilled nursing communities by promoting our outstanding clinical outcomes and patient-centered care. Using a value-based sales approach, you'll build strong relationships with hospital discharge planners, physicians, and other healthcare professionals to ensure seamless transitions into our communities.

Responsibilities

• Execute a targeted sales plan with monthly and quarterly goals
• Build and maintain key referral relationships
• Collaborate with internal teams to meet community census targets
• Earn monthly performance bonuses based on goal achievement

Requirements

• At least 2 years of experience in healthcare sales, business development, or clinical outreach
• Skilled nursing / long-term care experience preferred
• Strong communication skills, self-motivation, and ability to work independently
• Existing relationships in the Killeen/Harker Heights healthcare market are a big plus
