About The Position

High Point & Affiliated Organizations is a health and human service agency whose mission is to treat and prevent substance use disorders and mental illness. High Point has programs located throughout Southeastern Massachusetts offering a full continuum of care for substance use and mental health treatment, including inpatient, outpatient, residential, and community-based services. Programs and services also assist survivors of abuse, violence, and families experiencing homelessness. High Point believes that everyone has inherent goodness, worth, and dignity. Our goal is to help individuals and families achieve personal change and improve their quality of life. High Point’s Opioid Treatment Program (OTP) provides methadone treatment to adults age 18 and older with opioid use disorder. The OTP is focused on individualized treatment. Treatment plans are created with the patient and primary clinician and then reviewed by the multi-disciplinary treatment team with the medical director signing off. Populations served include but are not limited to men, women, pregnant women, patients with chronic illnesses, and dually diagnosed mental health disorders. This fast paced, early bird position that allows LPN’s to be in and out of work before many people get their day started!

  • Knowledge of Substance Use Disorders, Dually Diagnosed (Substance/Mental Health Disorders), Psychiatric Diagnosis and Medical Co-morbidities
  • Compliance with Federal Regulations regarding 42 CFR Part 2; Confidentiality of Substance Use Disorder and HIPAA Regulations
  • Adherence to Joint Commission National Patient Safety Goals for Behavioral Health
  • Adherence to DEA regulations and guidelines pertaining to controlled substances, including count, administration and documentation standards
  • Utilize critical thinking skills and apply best nursing practices in accordance with policy when making decisions and problem solving to provide for best patient care and outcome
  • Be familiar with and adhere to the rights of patients as defined by DPH Patient Rights Standards (105 CMR 164.079)
  • Be familiar with best practice standards in line with ASAM criteria inclusive of detox protocols, MAT and/or pharmacology
  • Documents patient information according to established alcohol/drug protocols as well as any psychiatric/medical conditions as per DPH Regulations indicate
  • Assist the patient in understanding the disease process through education and therapeutic relationship
  • Understands medication uses, side effects and food-drug interactions
  • Maintain therapeutic boundaries by setting limits and enforcing rules in a respectful manner
  • Utilizes effective time management
  • Collaborate with the interdisciplinary team
  • Triage patient issues for the Attending Practitioner
  • Responsible for HPAO continued education in the field of substance use disorders
  • Attends supervision with assigned supervisor twice monthly and/or as needed basis on status or concerns
  • Attend mandatory meetings and trainings
  • May be mandated to cover designated program/shifts based upon safe staffing needs
  • Perform any other duties as assigned by the Nurse Manager, Program Director, Site Director, and/or DON
  • Perform shift specific duties to ensure the unit is cleaned and properly stocked with necessary medical supplies
  • Complete Admission and Discharge Assessments and procedures as required
  • Monitor and treat patients who have co-occurring medical/psychiatric ilness
  • Assess the physiological and behavioral health status of the patient
  • Conduct controlled substance count and sign the controlled substance register
  • Monitor vital signs and assess withdrawal signs and symptoms per protocol
  • Administer routine medications according to the nursing standards of care as ordered by practitioner
  • Coordination of medication orders with pharmacies and alert practitioner when prior authorizations are required for medications
  • Conducts suicide risk assessment as needed and communicates information to practitioner
  • Review medications received at the program and log prior storing
  • Conduct a nursing assessment/physical assessment including medication reconciliation for new admissions and as requested by staff
  • Provide psycho-educational groups on medication awareness and effects as well as other subjects associated with addiction/mental illness, if required
  • Ability to engage in verbal de-escalation of patients and provide appropriate behavioral interventions to maintain safety
  • Maintain therapeutic boundaries by setting limits and enforcing rules in a respectful manner
  • Triage medication concerns for the attending practitioner
  • Complete all required shift tasks delegated for monitoring, auditing and compliance (logs, reports as assigned)
  • Audit unit supplies and submit orders as needed to supervisor
  • Engage in care coordination activities with the patient's community medical providers, as needed
  • Support patients with identifying medical service needs, assist with referrals and appointment coordination
  • Monitoring of medications stored onsite and following medication destruction policies as needed
  • Ensure exam room is maintained in cleanly manner, stocked with supplies and infection controls protocols are followed
  • Coordination with external laboratory to ensure patient lab orders are accurate, valid and samples are collected per policy
  • Complete required nursing documentation per policy for outpatient/OTP services
  • Report and environmental/safety concerns to supervisor immediately
  • Complete chart audits as requested to ensure documentation for nursing is completed timely.
