About The Position

Medical Providers at REACH Medical provide low-threshold, harm reduction-oriented medical care in a multi-disciplinary, integrated practice to further the REACH Mission of ensuring health equity to all who typically face stigma in the current healthcare system. This role is primarily a teaching role which requires working in various settings and educating addiction medicine fellows, internal medicine residents, physician assistant students, nurse practitioners and nursing students.

Requirements

  • Physician Board Certified in Addiction Medicine
  • NYS Medical License
  • Demonstration of both significant experience in and commitment to providing care to underserved populations as the foundation of REACH Medical is the provision of harm reduction-based, trauma-informed, low threshold care without stigma.
  • Solid communication skills and ability to work well within a multidisciplinary agency.
  • Deep understanding and belief in harm reduction, health equity, diversity and inclusion.

Responsibilities

  • Deliver collaborative, accessible, high quality primary care and MOUD to patients free of stigma
  • Provide medical care for patients and education to students, residents and fellows in the following possible locations: o REACH Medical - Ithaca NY o Street Outreach Ithaca, NY o Inpatient Consultative Services Cayuga Medical Center o Outreach and Mobile Services in Broome County, NY o Methadone Treatment as necessary at local OTP’s
  • Provide Medical care to include: o Screening for all substance use disorders utilizing medications for addiction treatment (MAT) in their care. o Acute primary care to include issues that patients would typically seek care for at an urgent care but limited to those that can be handled given the resources and presentation of the patient o Screening and treatment for mild to moderate behavioral health diagnoses primarily with the use of antidepressants such as SSRI’s Screening and treatment for hepatitis C and other viral illnesses often associated with drug use
  • Attend quarterly provider meetings via zoom
  • Attend all mandatory staff trainings
  • Work collaboratively with: o REACH contracted and employed medical providers and other program staff including but not limited to: Primary Care providers; Behavioral Health providers; Peer, Community Health Workers and Outreach staff; MAT providers. o Nursing staff o Front Desk staff o Care Management staff o Management team.
  • Abide by REACH standards of care for including: o Maintain required state and federal credentials and work with REACH and other needed credentialing team members to maintain needed credentials (for physicians this would include credentialing with the clinically integrated network affiliated with Cayuga Health System o Provider quality documentation in the EMR in a timely manner with the inclusion of appropriate medical billing o Complete 3-4 unique patient encounters per hour. o Flexibility to see an increased number of patients or add ins as needed
  • Share on average less than once weekly after hour call responsibilities (REACH has a triage system after hours so this task typically only requires signing prescriptions)
  • Share on average less than once weekly signing provider responsibilities which mirror on call in that all calls are screened and the provider typically signs bridge scripts for patients
  • Share after-hours call responsibilities with at least four other providers divided in proportion to hours worked.
