Hours: Full-Time, 40 hours/week The Addictions Counselor II serves as a counselor in inpatient or residential levels of care working with the substance use disorder (SUD) and co-occurring population. Plans, coordinates, and provides intervention, counseling, and case management services for inpatient or residential clients and their families.

Requirements

  • Master’s degree in counseling, Psychology or Social Work or related field.
  • Upon hire the individual will work toward licensure or certification.
  • One year of clinical experience providing care to patients with Co-occurring Disorder.

Nice To Haves

  • Previous experience in ASAM Criteria, evidence-based practices, discharge planning and case management models preferred.

Responsibilities

  • Meets with assigned clients within 24 business hours of admission to begin the process of developing a therapeutic relationship as well as the client’s treatment plan, and discharge plan.
  • Provides individual counseling for at least one hour per week.
  • Engages the family/supports in the client’s treatment, with consent of the client.
  • Conducts family sessions as clinically appropriate.
  • Facilitates group therapy sessions including those focused on co-occurring disorders, and educational groups, as assigned.
  • Reviews SUD and tobacco medication assisted treatment (MAT) options with the client and documents recommendations with clinical rationale, the client’s reaction and the plan for referral to the medical staff, if appropriate.
  • In conjunction with the client and aftercare specialist, develops a comprehensive discharge plan that includes therapeutic treatment for SUD at the appropriate level, therapeutic treatment for co-occurring psychiatric and/or medical conditions, and safe and appropriate housing.
  • Provides case management for assigned clients.
  • Completes all paperwork, in a manner consistent with professional, regulatory and payer standards, on time, to ensure proper documentation is in chart. This includes mental health screenings, treatment plans, progress notes, ASAM summary sheets, and discharge summaries.
  • Collaborates with other agencies, such as probation/parole and children and youth services, with the client’s permission, including preparing reports and correspondence as indicated.
  • Assesses clients and works with families in a professional, thorough and timely fashion, assuring that the client/family has a clear understanding of what is recommended, including alternative options, and the rationale behind the recommendation.
  • Triages clients who require services other than outpatient (i.e. Mobile Engagement Services (MES), Crisis, Mental Health services) in a timely and organized fashion and makes referrals as indicated.
  • Participates in daily morning report and able to share pertinent information about each assigned client, including but not limited to, the client’s insurance coverage, anticipated day of discharge, and anticipated discharge plan, including aftercare and housing arrangements.
  • Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information.
  • Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements.
  • Demonstrates/models the Network’s PCRAFT values during interactions with all customers.
  • Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety.
  • Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices.
  • Complies with Network and departmental policies regarding attendance and dress code.
  • Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.
  • Other duties as assigned.
