In this position, the ACM is responsible for assisting with writing letters and completing forms at the request of providers/therapists. The Administrative Case Manager assumes responsibility for assuring that residents in the defined geographic area receive appropriate and adequate mental health services and community support. Through screening, referral, and short-term/low intensity assistance, the incumbent is responsible for assisting individuals with mental illnesses in accessing federal and state financial entitlements, in coordinating treatment/rehabilitation services, and in linking to/acquiring community supports and resources needed to meet daily living/health needs. Not all administrative case managers are responsible for all position requirements.

Requirements

  • A high school diploma with one year experience working in a public or private human service organization.
  • Able to demonstrate understanding of principles of mental illness/mental health.
  • Able to learn and implement the principles of the Community Support Program (CSP) and Child and Adolescent Service System Program (CASSP).
  • Able to work with service participants and significant others in a manner that demonstrates a sincere respect and concern for the person’s welfare.
  • Must have and maintain a valid driver’s license accepted by the Network’s insurance carrier and maintain a good driving record.

Nice To Haves

  • A Bachelor’s degree with major course work in sociology, social work, psychology, gerontology, anthropology, criminal justice, theology, nursing, counseling, or education
  • A registered nurse
  • A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science, and two years’ experience in direct contact with mental health consumers
  • A high school diploma and five years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989.

Responsibilities

  • Assist service participants in applying for Medical Assistance, Supplemental Security Income (SSI), Social Security Disability (SSD), pharmaceutical Patient Assistance Programs, low-income housing and other resources/benefits as needed.
  • Function as a liaison for service participants: Interested in referral to and participating in educational/vocational services offered locally; Receiving treatment at area state and community hospitals.
  • Visit persons from defined geographic areas who are hospitalized, attend treatment planning meetings, and participate in discharge planning processes per county policy; Incarcerated at correctional facilities; assist in planning to ensure appropriate community support is in place prior to release.
  • Monitor treatment adherence of participants on involuntary outpatient commitments (IPOC).
  • Submit IOPC admission and termination information to appropriate county personnel as required.

  • Maintain community resource files and provide information and referral services to service participants, families and members of surrounding communities.
  • Attend scheduled meetings as required/needed.

  • Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information.
  • Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements.
  • Demonstrates/models the Network’s PCRAFT values during interactions with all customers.
  • Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety.
  • Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices.
  • Complies with Network and departmental policies regarding attendance and dress code.
  • Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.
  • Other duties as assigned.
