Rapid Rehousing for Young Adults is a solution to homelessness designed to help young adults aged 18-24 quickly exit homelessness to permanent housing. The program provides rental assistance and support services that are tailored to the unique needs of the young person. The Program Supervisor provides supervision and support to a unit of case managers, and other support staff to ensure the program participants receive high quality case management.

Requirements

  • MSW or master’s degree in a related field required
  • 2 to 5 years of experience providing case management services required
  • Experience working with homeless and at-risk youth required
  • Authorized to work in the U.S.
  • Ability to work in-person in New York City (NY) for at least 3 days or more per week.
  • Ability to travel to other Rising Ground sites (if required)

Responsibilities

  • Provide supervision and support to program support staff
  • Supervise the provision of case management
  • Conduct intake, screening and assessment of program participants
  • Review case notes, assessments, home visit, referral material data completed by case managers
  • Acts as a liaison with any existing service providers involved with the program participants
  • Participates in interdisciplinary treatment team meetings.
  • Facilitate Case Management Trainings, Workshops, and Mesetings.
  • Work with providers to obtain appropriate referrals.
  • Submit reports for the program.
  • Works with the Assistant Director & Director to develop and evaluate the program
  • Conducts outreach and presentations as needed and as assigned
  • Performs all other program and position duties as delegated
  • Conduct Home Visits with case managers at least quarterly for 50% of housed participants to ensure quality service
