Position Summary: Our admission nurse is the first point of contact between a patient and the healthcare center. The admission nurse performs a head-to-toe assessment, gathers important information, completes medication reconciliation, reviews medical history, and establishes communication process for seamless transition.

Requirements

2 years of LPN experience required.
Active LPN TN license or Active RN TN License

Responsibilities

Ability to communicate effectively with medical/health care community, patients and families.
Ability to organize and implement systems to manage and enhance admission patient services.
Maintains open and ongoing communication with patients and families, providing opportunity and encouragement to participate in decision making.
Monitors to see that treatments and medications are entered in electronic health records as ordered.
Participates in gathering and/or presenting information needed for level of care meetings and reports information to attending physicians as indicated.
Maintains a current knowledge of infection prevention and control and current standards of practice as well as local, state and federal regulations related to IPC practices in the center.
Performs other duties as assigned.
Relieves other shifts if needed.
