Facilitates patient admission, discharge, intra-hospital and inter-facility
transfer processes through clinical triage. Utilizes the electronic bed board to
assign patients beds to the appropriate clinical unit.
Requirements
Graduate from an accredited School of Nursing required.
Bachelors of Nursing, required.
Current License to practice as a Registered Professional Nurse in New York
State required, plus specialized certifications as needed.
1 year nursing experience in rehab or acute care facility, preferred.
Nice To Haves
Experience in clinical documentation review, preferred.
Responsibilities
Prioritizes and assigns beds based on appropriate level of care and patient
aggregation from the Emergency Department, Perioperative Services, MD offices
and patient transfers from other hospital.
Helps facilitate patient transfer to inpatient bed and ensures hand off
communication to accepting nurse.
Promotes optimum utilization of hospital beds by conducting rounds and
communicating with admitting staff, Emergency Department and patient care
units regarding bed availability, current census and admission needs.
Addresses and resolves issues arising from increased census and decreased bed
availability.
Promotes patient, family and physician satisfaction regarding bed
assignments.
Intervenes in conflicts as needed.
Liaises with Case Management to facilitate patient discharge, inter-facility
and intra-hospital transfer process.
