About The Position

The Adult Therapist | Nursing Home Liaison provides clinical services to consumers and families, with a primary focus on individuals residing in nursing homes. This role includes conducting assessments, developing and implementing treatment plans, delivering therapeutic services, and coordinating care across agencies. The position also supports educational, preventive, and case management functions to ensure consumers receive comprehensive, person-centered care.

Responsibilities

  • Conduct biopsychosocial assessments, intakes, and diagnostic evaluations within credentialed scope.
  • Develop, implement, and routinely review consumer treatment plans with active consumer and family participation.
  • Provide individual, group, and family therapy; crisis intervention; mental health consultation; and medication monitoring, as indicated by treatment plans.
  • Deliver services in the most accessible environment (home, school, emergency room, nursing home).
  • Support consumers in developing daily living skills and provide education to families.
  • Promote transition to less restrictive levels of care and meet/exceed productivity standards.
  • If certified, provide linkage, coordination, and crisis intervention for assigned caseloads.
  • Ensure compliance with targeted case management standards.
  • Assist consumers with medical appointments and maintain required monthly assessments.
  • Submit monthly data on nursing home days for Center-sponsored consumers.
  • Coordinate with JBS Mental Health Authority regarding non‑Medicaid‑covered expenses.
  • Complete documentation for Medicaid eligibility and Center sponsorship.
  • Collaborate with nursing home staff regarding potential admissions and placements.
  • Participate in staff meetings, clinical staffing, and peer reviews.
  • Collaborate with multidisciplinary team members including psychiatrists, nurses, and case managers.
  • Make and receive referrals to/from internal and external service providers.
  • Complete progress notes at time of service delivery unless clinically contraindicated.
  • Obtain required signatures and releases; prepare documentation for outside agencies.
  • Submit required Center, state, and federal forms with accuracy and timeliness.
  • Perform clerical tasks as assigned, including data entry.
  • Report safety hazards and participate in committees, workgroups, and special assignments.
  • May provide supervision to LPC candidates or student interns, as qualified.
