About The Position

The role of a Case Manager is to work with adults (ages 21 and up) to identify needs and goals that will strengthen the adult's future. Areas of focus include but are not limited to mental health, physical health, economic needs, education and positive social development. In this position, the Case Manager provides ongoing support and guidance to clients on a weekly basis, both within the community and in their homes. Transportation may be required to facilitate essential connections to community resources and services. Collaborating with other service providers and professionals is essential to ensure a comprehensive and integrated approach to client care. This challenging yet rewarding role is dedicated to changing generational cycles and enhancing the quality of lives in our communities. It consistently delivers some of the best outcomes in the field of human service work, making a significant impact on the individuals and families served.

Requirements

  • Minimum of 1 year adult case management/social work experience (or related experience) - volunteer/internship experience may be counted experience may be counted.
  • Minimum of 4-year degree (related human service degree preferred but not required)
  • Valid Driver's License for past 5 years
  • Ability to commute/relocate: Westminster (Carroll County), MD: Reliably commute or planning to relocate before starting work (Required)
  • Bachelor's (Required)
  • Day Shift (Required) - but will require some early evenings (at least 2-3 times a week)
  • First 2-4 weeks of training will be in our Hagerstown office (Required). Stipend would be provided - to be further discussed upon hire.

  • Potomac Community Services (PCS), due to its human services responsibilities and sensitive work, is obligated to maintain a Drug-Free Workplace.
