About The Position

Part-time and/or Full-Time Salary Range for Full-time: 150,000-185,000 KEY RESPONSIBILITIES Examines and assesses a consumer to determine general physical and behavioral health condition. Organizes data concerning consumer’s relevant history and current symptoms. Determines nature and extent of present disorders and formulates appropriate treatment response. Assesses patients with emergent and urgent care needs and makes determinations about safety in the community. Completes documentation and billing for all services in a timely manner and according to established standards. Achieves service volume standards as established by leadership based on daily hours worked and caseload size. Ongoing psychiatric management, including psychiatric medication management, psychotherapeutic support, and crisis intervention in collaboration with the treatment team and clinical supervisors; Educates clients about their illness and provides health management information and techniques Refers patients internally and externally for services as needed. Teams with physicians, other APRN’s, clinical supervisors, community support specialists, and administrative support staff to coordinate care. Completes administrative tasks as needed including timely response (24 hours or less) to voicemails, e-mails, communicating with external agencies and other treatment providers, etc. Documents clinical care and services provided; and Uses evidence-based standards, guidelines, and protocols to the delivery of patient care. Incorporates new information and approaches, when appropriate, to continuously improve patient care and practice. In addition to role responsibilities, each staff member of Community Connections has the following responsibilities as a part of their employment: Models and reinforces Community Connections mission to provide behavioral health, residential services, and primary health care coordination for marginalized and disenfranchised women, men, youth, and children living in the District of Columbia, many of whom are coping with challenges including mental illness, addiction, and the aftermath of trauma and abuse. Models and reinforces Community Connections values of quality, innovation, respect, equity, and integrity daily. Reinforces Community Connection’s commitment to diversity, equity, and inclusion. Protects the privacy of our consumer’s protected health information by maintaining compliance with HIPAA and other relevant CC related IT security regulations. Completes and stays current on role specific and organizational wide training. Performs other duties as assigned on an as-needed basis.

Requirements

  • Master’s Degree in Nursing required
  • Actively licensed by the DC Board of Nursing as an Advanced Practice Nurse Practitioner required
  • Minimum 2 years practicing as a Nurse Practitioner required
  • Active DEA licensure required
  • Active CDS licensure issued by the District required
  • Current BLS/ALCS/PALS
  • Licensure must be in good standing with DC Medicaid Board and be free from sanctions.
  • Psychiatric Mental Health Nurse Practitioner Board Certified is required.
  • Experienced in medication management required.
