About The Position

The emergency department Advance Practice Provider (APP) provides advanced nursing and patient care in the acute care setting based on educational preparation and/or certification in the practitioner role to a specific patient population. Care goals include promotion of healing, and prevention of illness and recidivism. The (APP) assumes the management of patients, functioning in both an independent and collaborative role with the physician, case manager, and other members of the health care team. As a practitioner, they perform select procedures, designs individualized treatment plans, evaluates outcomes, and modifies the treatment plan as needed in accordance with established management protocols and regulating bodies. The (APP) facilitates the transition of patients between service units and provides/coordinates complex patients and family teaching. This position is required to work weekends, nights and holidays as warranted. The delivery of professional nursing care at Stormont Vail Health is guided by Jean Watson's Theory of Human Caring and the theory of Shared governance, both of which are congruent with the mission, vision, and values of the organization.

Requirements

  • Master’s prepared with national certification eligibility.
  • Must be a graduate of an accredited PA/APRN program.
  • Meets educational preparation or other qualifications within specified time frames as determined/outlined by specific department policy.
  • All APRN incumbents are required to maintain Registered Nurse competencies as designated by Federal and State organizations and as outlined in SVH policy. (Required proficiency)
  • Registered Nurse - KSBN Must have valid APRN licensure.
  • Be certified as a nurse practitioner by a recognized national certifying body that has established standards for nurse practitioners. Required or Physician Assistant (PA) - KSBHA Required
  • Pediatric Advanced Life Support - PALS Required within 6 months of hire.
  • Advanced Cardiovascular Life Support - ACLS Required within 6 months of hire.
  • Advanced Trauma Life Support - ATLS Required within 1 year of hire.

Nice To Haves

  • Additional certification may be required in certain patient care settings or populations. The department director will provide specific information and the time frame in which such certification may be attained at the time of hire/transfer into department.

Responsibilities

  • Provides primary care for the emergency department/Trauma population in the outpatient setting.
  • Ensures that care delivered to patients is provided according to the Stormont Vail Health mission, strategic plans, and all internal/external regulations, policies and procedures.
  • Practices within the Kansas Nurse Practice Act and within the guidelines of management protocols.
  • Supports the established care models of Stormont Vail Health.
  • Functions independently and collaboratively with the physician and all disciplines to provide continuity of health care services for individuals, families, and/or groups.
  • Evaluates the health status, acute episodes of illness, and health care needs of individuals through emphasis on history, physical exam, and psychosocial assessment in collaboration with other health care teams.
  • Analyzes data to identify health/illness problems, the human responses to actual or potential health/illness problems, and variations from the norm in diagnosis, emergent problems, and complications.
  • Designs and evaluates individualized treatment plan.
  • Coordinates care for continuity of care across the health care continuum.
  • Attends emergency/trauma service rounds, participates in patient evaluation and care planning, reviews laboratory studies, vital signs and x-rays.
  • Intervenes as determined appropriate in collaboration with the emergency department/trauma team and within management protocols standards.
  • Under the supervision of the emergency department physician/trauma surgeon, writes and effectively communicates orders in accordance with management protocols.
  • Communicates new or altered findings to the emergency department physician/trauma surgeon.
  • Evaluates patients in the outpatient clinic setting, monitors problem development and provides appropriate referrals for follow-up and problem resolution.
  • Performs expanded role in the outpatient setting including suture, staple removal, wound care and other therapies in accordance with management protocols.
  • Updates patient’s chart and write dismissal orders according to the guidelines of the Stormont Vail Health Rules & Regulations.
  • Provides early identification of discharge needs and ensures that appropriate referrals are made with case management and social services, nursing care team and attending RN.
  • Consults with other members of the health care team to expedite disposition/resolution.
  • Collaborates with Trauma services team to improve care and achieve verification.
  • Facilitates referrals and specialty consults and assists in the coordination of care.
  • Facilitates patient/family participation in health care by identifying learning needs and providing education to enhance decision-making.
  • Assists staff in the coordination of a plan of care for the Emergency/Trauma Services patients.
  • After discussion of the plan of care with the Emergency Department physician/Trauma Surgeon, provides information to patients and their families regarding diagnosis, plan of care, expectations, and prognosis.
  • Provides telephone consultation to patients and/or physicians following discharge when on duty.
  • Assists in the development of research based clinical care protocols, guidelines, and pathways for emergency/trauma services patients.
  • Facilitates changes in clinical practice based upon document research.
  • Monitors and analyzes outcomes based on research driven practice change.
  • Assists staff nurses in participating in research and incorporating the findings into their practice.
  • Communicates with outreach physicians, hospitals, and EMS.
  • Refers emergency/Services patients to in-house support services and/or community services as needed.
  • Maintains education standards as established by management protocols.
  • Participates in professional education activities and attends national-level activities and attends national-level emergency service/trauma seminars.
  • Complies with all policies, standards, mandatory training and requirements of Stormont Vail Health
  • Performs other duties as assigned
