Advanced Practice Provider (NP/PA) - Outpatient ClinicPrime Healthcare | Affiliated with St. Mary of Nazareth HospitalLocation: Chicago, IL (Near-Northwest Side / Wicker Park) Prime Healthcare is currently seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our team at our Primary Care clinic in Chicago, Illinois. Position Highlights Prime Location: Practice in a walkable, energetic corridor with boutique shops, acclaimed restaurants, craft breweries, street art, live music, and cultural festivals. Practice Setting: Outpatient clinic environment, with seamless integration to hospital-based specialists, diagnostics, and ancillary services for coordinated patient care. Patient Population: Serve a vibrant, diverse community with a mix of preventive care, chronic disease management, acute visits, women's health, and minor procedures. Collaborative Model: Work closely with board-certified physicians, multidisciplinary teams, and support staff in a supportive, team-based atmosphere focused on quality outcomes and patient satisfaction. Requirements
- Licensed NP or PA in the state of Illinois
- Certification as a Nurse Practitioner or Physician Assistant
- Experience in primary care, family medicine, internal medicine, or urgent care is highly desirable

- Bilingual skills (Spanish or Polish) a plus, given our diverse patient population
Responsibilities
- Deliver full-spectrum primary care to patients of all ages
- Manage chronic conditions, perform preventative care, and address acute health concerns
- Work closely with physicians and clinical staff to ensure coordinated, high-quality care
- Maintain accurate and timely documentation in the electronic medical record
