About The Position

Under the general direction of the Chief Medical Officer, is responsible for the provision of a full range of primary care patient services consistent with the Scope of Practice allowed by the State of Maryland. Under the general supervision of the Chief Medical Officer, assists in the development, implementation and enforcement of clinical guidelines and participates as a member of the provider team available to MLMC patients.

Requirements

  • The APP shall be an experienced provider of primary care medical services.
  • he/she must be able to demonstrate an understanding of the administrative aspects of clinic operations and have a thorough grasp of the many and varied collaborative relationships necessary to maintain effective continuity of care for all MLMC patients.
  • The successful candidate must have the inter-personnel skills to establish and maintain effective relationships with professionals and patients at all levels and to assist the Chief Medical Officer in liaison with local, state and federal agencies when appropriate.
  • Must be duly licensed to practice in the State of Maryland as an APP.
  • Must be, or able to be certified in BLS.
  • Must be a graduate of a recognized Masters-level Nurse Practitioner/Physician Assistant program.
  • Must have the ability to follow HIPAA guidelines as well as an understanding of policies and procedures regarding medical records put in place by the Federal Government.
  • Must be able to demonstrate the ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • An essential element is the ability to work and interact effectively and positively with other staff members to build and enhance teamwork in a patient centered team based model of care and in the overall MLMC organization; team engagement is a must to be successful in this position.
  • A demonstrated work record showing good attendance, punctuality, dependability and the ability to work well with supervisors and coworkers as part of a team effort is essential.
  • A medical office setting is an environment that requires the ability to relate to all types of people while always maintaining a professional demeanor.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals is essential.
  • Ability to write routine reports and correspondence, and the ability to speak effectively to patients, co-workers and the public at large is paramount.
  • Must have the ability to read and interpret medical and work environment related documents involving patient care, program descriptions, government regulations and MLMC policy and procedures.
  • Must be able to demonstrate an understanding and sensitivity to cultural differences and needs of MLMC patients and be able to respond appropriately to all people regardless of race, ethnicity, language, age, gender, sexual orientation, religion, disability, economic standing, etc.

Nice To Haves

  • A minimum of two (2) years experience in direct provision of primary care services is preferred.
  • Additional experience is very desirable, particularly in the setting of a community health care center.
  • Well-developed interpersonal skills as they relate to fellow MLMC staff and patients in particular and the local medical community and public in general.
  • Certification in ACLS, ATLS and PALS is encouraged.

Responsibilities

  • Work effectively with the Chief Medical Officer and other members of the support staff in the provision of high quality medical care within a team environment.
  • The APP (Nurse Practitioner or Physician Assistant) will manage a panel of patients and be responsible for their full range of medical needs.
  • Provide a full range of primary care services in the outpatient setting and assist in the referral of patients to other outside services when appropriate.
  • Cooperate with all provider staff in the conduct of a meaningful and productive peer review process; must meet all quality measure and documentation requirements.
  • Works respectfully and effectively with executive and leadership personnel in serving MLMCs mission and accomplishment of the strategic plan and organizational goals.
  • Timely correspondence as necessary through email, phone, or face-to-face exchange of information to accomplish the goal of team based, patient centered care.
  • Attendance at Provider Meetings and Town Hall meetings (excused when on approved leave).
  • Additionally, held accountable for committee and team responsibilities and duties sometimes assigned by CMO or senior leadership when applicable.
  • Participate with other MLMC staff in outreach efforts which may involve health fairs, in-school clinics or educational programs and other similar community functions.
  • Promote MLMC and its services to the community in general, the medical community in particular and the many and varied constituents.
  • Work with other MLMC staff and services in a comprehensive patient treatment process as part of an overall plan of care to disease prevention and treatment.
  • Assist the Chief Medical Officer and MLMC executive staff wherever possible in pursuit of additional grant and funding opportunities consistent with the mission of MLMC in order to expand the centers capacity to meet the health needs of the community.
  • Participation in activities needed to execute the goals, objectives, and initiatives of such grants and funding.
  • Comply with the established clinical policies and procedures that address established standards of medical care and ensure continuity of care that focuses on all the health care needs of MLMC patients.
  • Participation in MLMC programs and contractual agreements with various payers, organizations, and associations that promote, quality, team based care, and patient centered initiatives. Examples include, but not limited to Accountable Care Organizations, PCMH recognition, projects specific to grant agreements, payer value-based contracts.
  • Timely completion of charts and all patient care documentation as defined by MLMC protocols and policies (example: charting completed prior to EHR encounter locking)
  • Timely addressing patient phone messages and diagnostic and lab results through the EHR.
  • Provides excellent patient centered customer service while being friendly and courteous to all employees and patients; Handle multiple tasks at once and has the ability to compassionately and empathetically care for patients.
  • Complies with the terms of the Employment Agreement
  • Upholds the Code of Ethics and mission of Mountain Laurel Medical Center in a manner consistent with the core value of patient focused, respect, accountability, teamwork, continuous improvement, and integrity.
  • Perform such other duties and assume such other responsibilities as may be assigned by the CMO or CEO.
