About The Position

Provide on-call services after hours, and complete skilled visits including scheduled visits and emergent needs. Coordinate hospice care by working with other team members as well as with the patient's family members to ensure needs are met. Triage calls, coordinate services, and provides frequent updates on plan of care and any changes in condition. Assist reinforcement of patient and family education regarding end-of-life care. Participate in IDG and case conferences when needed Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning. Keep the RNCM informed of changes noted during the process. Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research reading. Respond to on-call or medical emergencies (nights and weekends)

Requirements

  • Licensed RN in state of practice
  • A minimum of 1 year of Clinical experience
  • Valid Drivers License and Auto insurance Coverage
  • Reliable transportation, willingness to travel
  • On-call/ triage experience preferred
