About The Position

Mission Hospice & Home Care serves patients and families in the San Francisco Peninsula and South Bay with quality care and compassionate end-of-life support and education. Founded in 1979, we are a local, independent nonprofit that has provided thousands of patients and their families with physical, emotional, and spiritual support throughout the journey from a life-threatening diagnosis through death and bereavement. We are seeking a per diem After-Hours Nurse to provi­de skilled nursing and hospice care to maximize the comfort and health of patients. Schedule: Wednesday - Sunday; 4:00pm - 12:30am Location: San Mateo County with additional coverage to Contra Costa County and some parts of Alameda County Mission Hospice & Home Care follows all CDPH vaccine requirements for healthcare personnel. Requirements

  • One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing.
  • Current license as a registered professional nurse in the State of California.

Nice To Haves

  • Experience in acute care/ home health and/or hospice setting preferred.
  • Public Health Nursing certification highly desirable.
  • HPNA certification preferred.

Responsibilities

  • Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
  • Under the guidance of the After-Hours Nurse, responsible for patients/families including assessing, planning, implementing, and evaluating nursing services consistent with the plan of care.
  • Communicate with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/ family.
  • Maintain regular communication with Team Leaders regarding patient/family status and unusual or potentially problematic patient/family issues.
