About The Position

Join our Shields team today! Shields Health Solutions is redefining patient care through specialty pharmacy. We’re hiring an Ambulatory Clinical Pharmacist—a pharmacist who will play a hands-on role in elevating patient care. In this dynamic, patient‑centered position, you’ll build meaningful relationships with patients and providers while delivering high‑quality medication management across diverse disease states. Working remotely alongside an engaged clinical team, you’ll conduct virtual assessments, optimize therapy, resolve medication‑related problems, and champion evidence‑based care.

Requirements

  • Hold a Doctor of Pharmacy (PharmD)
  • Must be licensed pharmacist in CA
  • Possess certification in a relevant field of practice (e.g. Board Certification)
  • Completed a postgraduate residency through an accredited postgraduate institution where at least 50% of the experience included the provision of direct patient care services with interdisciplinary teams
  • Have provided clinical services to patients for at least one year under a collaborative practice agreement or protocol with a physician, advanced practice pharmacist, pharmacist practicing collaborative drug therapy management, hospital pharmacy practice or health system
  • 3+ years' experience as a clinical pharmacist or completion of PGY2
  • Strong communication, organization, and tech skills
  • Empathy, integrity, and a team-first mindset

Responsibilities

  • Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug‑level assessments aligned with evidence‑based standards.
  • Partner with physicians, specialists, and interdisciplinary teams to deliver individualized care plans and seamless medication coordination.
  • Monitor therapy effectiveness and safety, identify optimization and cost‑saving opportunities, and support quality‑improvement and REMS initiatives.
  • Streamline operations by supporting complex prior authorizations, improving workflows, educating patients, and maintaining ongoing clinical development.
