GENERAL PURPOSE: In an outpatient setting, remains accountable for provision of direct care to assigned patients. Contributes to unit activities and supports enhancement of the care delivery system. Works as a team member and assists other staff members with implementing patient care. Assumes responsibility of care for assigned patients under the direction of the co-assigned RN or MD. Participates in the plan of care, implements appropriate nursing interventions, and contributes to patient/family education through collection of data, interdisciplinary team collaboration, documentation and excellent communication. FT (40) Monday-Friday business hours, no call, weekends or holidays. Seeking a compassionate LPN looking to help care for our infectious disease patients, seen on AC3 by our interdisciplinary team. This role will provide injections for treatment as well as preventative health care needs. Our nurse will also assist with wound care, monitoring patients undergoing IV therapy and small, in office procedures as needed. Looking for strong communication, collaboration and EMR skills. LPN will be integral to the care of this specialized patient population.
