About The Position

Position Summary In a collaborative and multidisciplinary environment, contributes professional and clinical expertise to assigned unit(s) and/or patient populations (Palliative Care) to ensure the delivery of high-quality care. 1. Expert Clinical Practice: In collaboration with collaborating/supervising physician, assists in all aspects of inpatient , outpatient, facility, and home care for a select patient population. Maintains expertise in area of practice and applies advanced clinical skills to care for that select patient population. Incorporates advances in technology and procedures into skill set, remains current in practice of palliative care. Assist in the evaluation of outcomes and adjustment of practices to ensure optimum delivery of care in a cost-effective manner. Facilitate the coordination and the communication of patient care needs across the health care continuum. For specific duties and responsibilities, see below. 2. Expert Guidance and Coaching: In collaboration with the collaborating/supervising physician, implements a therapeutic comprehensive plan of care for patients and families monitoring all aspects of the therapeutic regimen. Educates patients and families in prescribed treatment plan of care. Educates and guides families to promote wellness and assume responsibility for participation in the management of their own health care. Educates patients and families to facilitate understanding of the patient's medical condition and proposed course of therapeutic intervention. Serves as a resource person for education of health care professionals and the community-at-large to promote health maintenance and disease prevention.3. Consultation/Collaboration: Serves as a clinical resource and expert throughout the institution in area of specialty; maintains knowledge of system operations to expedite unit needs (e.g. who to contact, best methodology to accomplish desired result). Responds to consultative requests consistent with the directions of the supervising physician and provides recommendations for clinical/educational/psychosocial interventions as necessary. Collaborate with multidisciplinary care team as needed to optimize quality and delivery of care.4. Research: In collaboration with multidisciplinary staff, supports research initiatives of the organization through education, practice, and leadership. Critically evaluates existing practice based on current research findings and integrates changes into practice. Incorporate research findings and quality improvement strategies into clinical practice to improve patient outcomes. Participates in research projects as appropriate, maintains documentation and databases to support projects.5. Leadership: Act as a professional role model to all levels of staff by incorporating expertise and related experience into all aspects of patient care. Foster communication between members of multidisciplinary care team. Identifies the need for new programs, assesses potential outcomes and cost/benefits, and makes recommendations for further development, implementation, and evaluation. Works with other departments to establish new programs for patient care delivery, assessment, and evaluation. Monitors practice patterns for trends in outcomes and resource utilization for patient populations in area of practice. Education Graduate of an accredited Physician Assistant program as determined by the Committee on Allied Health. Education and Accreditation or a current Registered Professional Nurse and Graduate of an accredited Nurse Practitioner Program/master’s in nursing. Current Certification from the National Commission on the Certification of Physician Assistants (NCCPA) or for Advanced Practice Nurses a current Board Certification from the American Nurses Credentialing Center (ANCC) or from the American Academy of Nurse Practitioners (AANP). Experience Minimum of 3-5 years of clinical nursing practice experience, primarily in area of specialty. Skills Hold a valid, current New Jersey license. Obtain a New Jersey CDS and federal DEA certificate upon employment if not already covered. Evidence of excellent interpersonal skills, effective communication skills, and creative problem solving. Participation in pertinent professional and/or specialty organizations preferred. BLS certification . Driver`s License - current and valid driver`s license, registration, inspection sticker and appropriate level of insurance coverage required, to include collision. Employees must adhere to all procedural guidelines in HR Policy 108 including demonstrating an acceptable driving record for the duration of their employment as long as driving continues to be a responsibility of the job.

Requirements

  • Graduate of an accredited Physician Assistant program as determined by the Committee on Allied Health. Education and Accreditation or a current Registered Professional Nurse and Graduate of an accredited Nurse Practitioner Program/master’s in nursing.
  • Current Certification from the National Commission on the Certification of Physician Assistants (NCCPA) or for Advanced Practice Nurses a current Board Certification from the American Nurses Credentialing Center (ANCC) or from the American Academy of Nurse Practitioners (AANP).
  • Minimum of 3-5 years of clinical nursing practice experience, primarily in area of specialty.
  • Hold a valid, current New Jersey license.
  • Obtain a New Jersey CDS and federal DEA certificate upon employment if not already covered.
  • Evidence of excellent interpersonal skills, effective communication skills, and creative problem solving.
  • BLS certification
