The 297-bed facility offers opportunities in specialized nursing units, including Short-term Rehabilitation and Long-Term Care in a warm and caring environment The Assistant Director of Nursing (ADON) at The Commons on St. Anthony oversees the day-to-day functions of the nursing department, in accordance with current rules, regulations and guidelines under the guidance of the Director of Nursing and facility Administrator. The ADON ensures that residents receive the highest degree of quality care at all times. Pay Range: $113,000 - $120,000 anually

Requirements

  • 8+ years’ experience in an acute care, long-term care or similar facility desired.
  • 5+ years’ experience in nursing management or leadership desired.
  • Graduate of an accredited School of Nursing. BS in Nursing or related field required.
  • Current New York State RN license and registration in good standing.
  • CPR certified.
  • Current knowledge of best practice nursing standards and the care planning process.
  • Proven patient assessment and clinical skills.
  • Knowledge of applicable laws, guidelines and NY DOH regulations.
  • Ability to manage, motivate and guide staff while carrying out the duties of an RN.
  • Ability and knowledge to respond in emergency situations.
  • Outstanding interpersonal skills including the ability to communicate effectively with patients, residents, and families from diverse backgrounds who may have physical, sensory and mental impairments.
  • Strategic planning capabilities.
  • Proven decision-making, organizational and time management skills.
  • Ability to analyze problems, plan resolution strategies, implement solutions and evaluate outcomes.
  • Effective conflict resolution skills.
  • Ability to be available 24/7 to handle on-call responsibilities.
  • Proficient in MS Office, EMR and able to learn new software rapidly.
  • An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.

Responsibilities

  • With the Director, coordinates nursing practices to assure the delivery of safe, effective and efficient care and services by staff.
  • Directs resident care planning processes, meetings, updates and changes as necessary.
  • Ensures resident care plans are developed, communicated and followed through.
  • Reviews nurse’s notes and other documentation tools.
  • Determines the staffing needs of the nursing services department, and develops work schedules to meet the total nursing needs of the residents.
  • Acts as an integral part of and collaborates with the Interdisciplinary Team (IDT) with a goal of providing optimal care.
  • Actively rounds on all floors daily. Oversees clinical practice managers and delegates responsibility fairly. May provide direct nursing care if necessary.
  • Supports residents and families as needed.
  • Oversees medication management and delivery systems.
  • Collaborates with the Administrator, nursing team, and medical providers to maintain or improve existing processes and standards of practice.
  • Ensures the facility complies with DOH and other applicable regulations at all times and takes corrective action when needed.
  • Interprets policy and regulations for staff. Ensures understanding and compliance. Addresses non-compliance issues immediately.
  • Participates in making recommended changes to the Nursing Service Procedures Manual in accordance with acceptable standards of practice. Implements and communicates updates per DOH, CMS and other regulatory agencies.
  • Coordinates and ensures the successful completion of all PRI and MDS assessments within the mandated timeframes.
  • Assists in directing the processes that ensure accurate and complete documentation for forms, reports, charts, assessments, evaluations, etc.
  • Actively participates in and promotes the facility's quality assurance performance improvement program (QAPI) for the nursing service department. Monitors and implements quality measures and best practice standards. Serves as an active member of the QAPI Committee.
  • Responsible for incident reporting and related record keeping. Assures the completion of accurate recording, investigation and tracking of all incident reports for the facility. Submits incident report information on the Department of Health website as required and provides follow-up to completion including correction plans.
  • Plays an active role in the reporting of adverse outcomes for residents for the regulatory process.
  • Actively participates in utilization reviews and corrective action.
  • Ensures the prompt investigation and resolution of complaints from clients, families, physicians, contractors, DOH, and employees.
  • With the Director, is responsible for the success of mock facility surveys (inspections) in preparation for government agency surveys.
  • With the Director, provides support to DOH during annual survey or other third party audits.
  • May participate in writing plans of correction for DOH surveys, if indicated, and supports an educational plan for correction within the regulatory time frame.
  • Determines the staffing needs of the Nursing Department to ensure quality patient care standards and needs are met. Monitors performance standards (absenteeism, care activity, vacation schedule, etc.) to ensure that an adequate number of nursing care staff are on duty at all times.
  • Monitors nursing service staff to assure that they follow the established safety regulations in the use of equipment and supplies.
  • Ensures staff receive supervision and education in care delivery, safety, aseptic, infection control and isolation techniques based on their role.
  • Ensures that all resident care rooms, treatment areas, service work areas etc. are maintained in a clean, safe and sanitary manner.
  • Keeps self and staff current in best practices nursing care. Participates and encourages participation in professional associations, educational conferences, and in-services as appropriate or required.
  • Compiles and presents written evaluations of the nursing program as needed.
  • Collaborates with the interdisciplinary team in developing safety and best practice standards for the Nursing Service department.
  • Monitors and manages budget lines, taking remedial action as required. Oversees the scheduling of all nursing staff using resources such as agency staff prudently. Works within budgetary constraints and resources without causing unnecessary overtime. Participates in budget development.
  • In the absence of the Administrator or Director of Nursing, directs the resident care policies established by the facility.
  • As a Director, this individual has full responsibility for an organization-wide function or service.
  • Recognized as the in-house expert or lead person for the function or site. Develops and executes strategy, policy and tactics for the site or function. Keeps current in his/her area of specialization, applicable regulations and in management trends.
  • Demonstrates effective leadership skills of delegation, organization, and coordination in managing the daily operations of their function or site, including performance management, scheduling, orientation, and training. In partnership with Human Resources, makes or approves recommendations on employee hires, transfers, promotions, salary changes, disciplinary actions, terminations, and similar actions. Resolves grievances and other personnel problems in a fair, timely and consistent manner, also in partnership with HR.
  • Promotes and encourages teamwork among employees and managers, and between departments. Communicates ideas and goals clearly and is an effective listener. Ensures that information is efficiently communicated and shared throughout the function and organization. Fosters constructive feedback and effective coaching.
