About The Position

Supports patients with dementia and their caregivers. Experience with ageing population care and caregivers is preferred. Key Expectations Within 12 Months: Effectively screen, assess, and onboard patients into the program. Maintain and manage an active patient caseload. Serve as a resource and support partner to team members to ensure high-quality service for the program population.

Requirements

  • Bachelor’s degree or equivalent years of relevant experience.
  • 0–3 years of experience.
  • Ability to work in a dynamic and complex business environment.
  • Demonstrated ability to manage multiple tasks, events, or projects simultaneously.
  • Strong communication and interpersonal skills .
  • Effective organization and time management abilities to balance outreach and patient management tasks.
  • Comfort working in a newer, evolving program with flexibility and a learning mindset.

Nice To Haves

  • Experience with ageing population care and caregivers is preferred.

Responsibilities

  • Serve as a bridge between the community and healthcare, government, and social service systems.
  • Assist individuals, families, and groups in accessing resources; provide informal counseling, support, and follow-up .
  • Coordinate frequently with CMS GUIDE Model team members and IU Health providers on behalf of patients.
  • Support community outreach strategies, initiatives, and events that meet identified community needs.
