Job Description DUTIES AND RESPONSIBILITIES: The incident/quality/risk manager is responsible for the following duties. Each section will have the duties separate although many aspects of this position will cross the boundaries of the other section of the job. CRITICAL ELEMENTS: Incident Manager: 1) Carry HCSIS beeper. 2) Answer calls on HCSIS beeper. a) Provide direction to staff to appropriately deal with the incident to ensure that the individual’s health, safety and wellbeing are addressed. 3) Ensure all incident reports are completed on HCSIS within the reporting time frame. This will include: a) An initial report to HCSIS within 24 (twenty-four) hours of incident occurring (72 [seventy-two] hours of medication errors and restraints.) b) Complete “final report” within 30 (thirty) days of incident. c) Follow through with incident report to ensure the incident is “Closed (i.e. approved by the county and the state)”. 4) Keep paper copies of incident reports in the office and send copies to the group home for the individual’s ISP book. 5) Contact family members, legal guardians or support coordinator of the occurrence of an incident as applicable. 6) Ensure notification requirements of 35 P.S. 10225.101 – 10225.5102 and 23 PA C.S. 6301 – 6384 (relating to Older Adult Protection Service Act and Child Protective Services Law) are met. 7) Complete investigations within the 10 (ten) day time frame outlined by Office of Developmental Programs Incident Management Bulletin. 8) Ensure paperwork for Peer Reviews (formal) is completed for each investigations and paperwork is kept with investigation. 9) Supervise other Certified Investigators (CI). a) Assign at least one (1) investigation per year to other CI’s. b) Offer assistance as needed. c) Ensure the CI follows the 10 (ten) day time line. 10) Participate in review of investigations to determine outcome. a) Either present investigation if CI on record or, b) Review investigation if no CI on record. 11) Complete semi-annual report to the county as per ODP’s IM Bulletin by June the 2nd (second) Friday of June and December for the 2 (two) reporting quarters. a) June – 4th (fourth) and 1st (first) quarter. b) December – 2nd (second) and 3rd (third) quarter. 12) Implement Bell’s ID Department Incident Management Policy. 13) Complete disciplinary action with staff in regards to incidents where staff broke policy and procedure (i.e., medication error.) 14) Complete Incident Management Training on a yearly basis with all staff in the ID Residential Program and other programs within the department as needed. Quality Risk Manager: 1) Schedule meeting with staff to “de-brief” after an incident has occurred to review if: A different outcome could have occurred. All policies and procedures were followed. Individual’s ISP and behavioral support plans (if applicable) were followed. Develop plans of correction to incidents with the Director of ID Services, or Coordinator of Residential Services along with Program Coordinator/Supervisor of program and any other staff member that is part of the individuals team who you feel is needed to successfully develop a thorough plan. Implement plans of correction to incidents by training staff on plan. Ensure that any safety concern is addressed accordingly and refer incident to safety committee for review. Other: 1) Attend monthly County Medication Task Force Meetings. 2) Maintain a working professional relationship with the County IM Reviewer and the ODP IM Reviewer. 3) Assist the Training Coordinator to complete Monthly Review of MAR’s for accuracy and make any necessary changes to bring paperwork into compliance. 4) Complete quarterly MAR reviews for staff as per the Training Coordinator. 5) Ensure all supplies necessary to do an investigation are available to all investigators. 6) Be familiar with Bell’s policies and procedures and Chapter 6400, 6500 and 2380 State Licensing Regulations as well as the philosophical beliefs of the Agency. ADDITIONAL ELEMENTS: 1) Follow through on assigned tasks. 2) Show ways to improve the incident management process within Bell. 3) Attend 24 (twenty-four) hours of training each year in addition to all mandatory training. 4) Attend any meeting related to incident management. 5) Other duties as assigned.
