About The Position

You will serve as the coordinator for the Asthma Prevention and Control Program. You will be primarily responsible for coordinating asthma prevention activities in the following strategic focus areas: Expanding access to and delivery of asthma self-management education and home visits for asthma trigger reduction. Reducing smoking and exposure to secondhand smoke. Increasing use of guidelines-based medical management. Strengthening linkages and coordination of care across settings. Introducing best practices and environmental policies to reduce asthma triggers from indoor, outdoor, and occupational sources. You will also act as a liaison between the Asthma Prevention and Control Program and the Tobacco Prevention and Control Program, ensuring activities are coordinated and supported between programs.

Requirements

• A Bachelor’s Degree in Public/Community Health, Health Administration, Health Sciences or Health Education; Public Administration, Government, Social Work, or a closely related field and three (3) years professional experience in the planning, development, coordination and/or evaluation of health programs or services -OR- an equivalent seven (7) year combination of education, training, and experience.

• Knowledge and experience with chronic disease prevention, management, and/or coordination of chronic disease programs.
• Experience developing health communication materials.
• The ability to understand and act upon surveillance and evaluation data to improve program operations.

Responsibilities

• Work as part of a team to execute asthma prevention strategies, inclusive of work plan activities funded by the U.S. CDC Advancing Health Equity in Asthma Control through EXHALE Strategies cooperative agreement.
• Participate in work plan development, coordinate the implementation of planned activities, and oversee related contracts.
• Oversee development and dissemination of communication materials related to asthma prevention.
• Lead workgroup efforts to promote healthy homes by addressing household environmental health risks.
• Coordinate and facilitate partner meetings focused on the planning and implementation of the indoor air quality monitoring pilot project.
• Coordinate with stakeholders, contractors, and other Maine CDC programs to implement a comprehensive approach to asthma prevention.
• Initiate and sustain partnerships with organizations that support program goals.
