About The Position

The Behavioral Health Case Manager is responsible for coordinating and delivering comprehensive, client-centered care that improves health outcomes and quality of life for individuals with behavioral health conditions, substance use disorders, and co-occurring diagnoses. This role focuses on addressing both clinical needs and social determinants of health through a holistic and compassionate approach. The Case Manager supports Behavioral Health Therapists by providing approximately 50% direct client care and 50% care coordination. Responsibilities include linking clients to community-based resources, facilitating referrals, responding to information requests, participating in multidisciplinary and community-based meetings (such as staffing conferences), and assisting clients with connections to specialized criminal justice dockets and other supportive systems. This position requires a strong foundation in behavioral health care, including treatment approaches for mental health and substance use conditions. The ideal candidate will demonstrate excellent communication, critical thinking, and problem-solving skills, as well as the ability to remain calm and effective in high-stress situations. A solid understanding of professional ethics, HIPAA regulations, and compliance requirements is essential.

Requirements

  • Licensed Social Worker (LSW) or eligible for licensure as a Social Work Assistant (SWA) in the state of Ohio
  • Demonstrated understanding of behavioral health, substance use disorders, and co-occurring conditions
  • Knowledge of how culture, identity, and lived experience impact care delivery and engagement
  • Strong interpersonal skills with the ability to engage diverse populations in a respectful, empathetic, and trauma-informed manner
  • Excellent written and verbal communication skills
  • Strong organizational skills, with the ability to manage multiple priorities and documentation requirements
  • Proficient in care coordination, community resource navigation, and client advocacy

Nice To Haves

  • Licensed Professional Counselor (LPC), Certified Case Manager (CCM), or Licensed Chemical Dependency Counselor (LCDC), preferred
  • Designation as a Qualified Mental Health Professional (QMHP), preferred
  • Bilingual in English and Spanish (preferred but not required)

Responsibilities

  • Conduct initial screenings and assessments to identify clients' behavioral health needs and barriers related to social determinants of health, delivering services in a timely, effective, client-centered, and trauma-informed manner.
  • Support clients in achieving wellness, autonomy, and improved quality of life by facilitating informed decision-making regarding their diagnosis and treatment options.
  • Coordinate and monitor all aspects of client care, including care planning, service referrals, and interdisciplinary communication.
  • Collaborate closely with Behavioral Health Therapists and medical team members throughout the case management process to develop and update individualized care plans.
  • Track and monitor client engagement, referrals, service utilization, and outcomes to ensure care plan goals are met.
  • Identify and address barriers to care by connecting clients to appropriate community-based resources, programs, and services.
  • Serve as a liaison between clients, families, providers, legal system personnel, and external agencies (e.g., CPS, criminal justice dockets), ensuring coordinated and comprehensive care.
  • Support client understanding of their behavioral health diagnosis, treatment plans, and community resource options.
  • Provide psychosocial support and brief interventions to address motivational, emotional, and behavioral concerns.
  • Accurately complete documentation and case notes in accordance with agency protocols and regulatory requirements; assist in preparing reports and tracking metrics.
  • Develop and maintain collaborative relationships with community-based organizations and care networks to enhance referral pathways and client outcomes.
  • Uphold ethical standards, client confidentiality, and HIPAA compliance in all interactions and decision-making processes.
  • Demonstrate strong written and verbal communication skills, including the ability to engage clients, advocate effectively, and convey clinical information to internal and external stakeholders.
  • Apply critical thinking and sound judgment in crisis intervention, problem-solving, and ongoing client care coordination.
  • Maintain a strong knowledge base of behavioral health, substance use, and social service systems to support the needs of a clinically and socially complex population.
