Join MCHC Health Centers as a Behavioral Health Community Health Worker (CHW) and be part of a team dedicated to making a difference in our patients’ lives every day. In this rewarding role, you’ll help improve access to care for patients with mental health and substance use-related diagnoses by offering compassionate support, advocacy, and care coordination.
As a CHW, you’ll connect patients with vital resources within MCHC and the community, facilitate referrals for behavioral health services, and work closely with our Enhanced Care Management (ECM) team, case managers, and healthcare providers. Your role will be essential in bridging gaps in care, ensuring patients feel supported, and improving health outcomes.

Requirements
Required Qualifications:
High school diploma or equivalent.
Bilingual (English /Spanish) required
Strong interpersonal and motivational interviewing skills.
Preferred Qualifications:
Associate’s degree in a behavioral health, health, or social services-related field.
Completion of a formal Community Health Worker training program.
Experience in behavioral health, case management, community outreach, and working with underserved populations.
Familiarity with social determinants of health and community resources.
Experience with electronic medical record (EMR) systems, such as Epic.
