About The Position

As an Medical Assistant on our ProHealth Integrated Care Team, you are the bridge that connects physical and mental health care. You’ll work with behavioral health clinicians, case managers, primary care providers, and nurses to deliver seamless, compassionate, wraparound support for every patient you work with. This is not a routine clinic job—this is a unique opportunity to change lives.

Requirements

  • Willingness to Learn: No prior experience? No problem! We’ll teach you everything you need to know.
  • Graduate of an accredited Medical Assistant program, current CPR and BLS certification.

Responsibilities

  • Patient Care Support: Meet patients in the community (homes, shelters, partner agencies, etc.) to support engagement and reduce barriers to care. Assist patients with Specialist care coordination, navigating transportation barriers, Blood Pressure monitoring, Medication support FSS, Chronic Illness education, Motivational support to manage health and any other barriers to receiving care. Support during and after provider visits by helping patients to prepare, review, and follow through with provider recommendations. Conduct outreach calls to patients for follow-ups, missed appointments, and check-ins.
  • Administrative duties: Medication reconciliation, Maintaining the Diabetes and Hypertension registries, sending refill requests to the providers, and scheduling appointments.
  • Team Collaboration: Work closely with the behavioral health and primary care team to address both physical and mental health needs, ensure coordinated care, participate in team meetings to discuss progress and treatment plans, provide feedback on progress towards physical Health and wellness goals as well as provide observations to the clinical team regarding patient behavior and needs.
