About The Position

The Behavioral Health Nurse Practitioner provides therapeutic and medication management services to patients with mental health and substance use disorders at Clinics of Duval Family Health Center. He/she works under a protocol with the clinic psychiatrist(s) to develop and implement an appropriate treatment plan. The Nurse Practitioner actively collaborates with other healthcare providers to ensure holistic and integrated patient care, and maintains documentation and patient records in accordance with regulatory and Center policies. The Behavioral Health Nurse Practitioner is sensitive to cultural, spiritual and practical needs of patients, residents, families, guests and staff members in all interactions. He/she provides positive feedback and comments about the quality of work and effort that have been offered by patients, residents, families and coworkers, and adjusts priorities and schedules to meet specific customer needs. The Nurse Practitioner reports and/or corrects problems related to safety, cleanliness and comfort of resident, patient and work areas, and follows Universal Precautions and Infection Control techniques. The position demonstrates an understanding of their role in facility safety plans and procedures related to fire, weather and other disasters.

Requirements

  • ARNP unrestricted license to practice in the state of Florida
  • Minimum 2 years experience prescribing in behavioral health setting
  • ANCC certification in psychiatry
  • State active DEA or ability to obtain
  • Excellent interpersonal and time management skills
  • Customer service focused
  • Excellent written and oral communication skills
  • Transportation and valid Florida driver’s license
  • Physically able to work in a fast-paced environment
  • Ability to travel locally
  • Be able to provide high-quality, person-centered care in a variety of settings
  • Flexibility
  • Ability to lift 20 lbs

Nice To Haves

  • Precepting experience preferred
  • Prior work with clients with a history of homelessness, severe and persistent mental illness and substance use disorders preferred

Responsibilities

  • Provide psychoeducation and appropriate therapies to individuals suffering from mental health and substance use disorders, including medication assisted treatment.
  • Develop and implement treatment plans based on clinical experience and knowledge.
  • Refer patients, clients, or family members to community resources or to specialists as necessary.
  • Complete all relevant charting, documentation and follow-up.
  • Fill out and maintain client-related paperwork, including federal- and state-mandated forms, client diagnostic records, and progress notes.
  • Communicate with families, probation officers, police, or other interested parties to exchange necessary information during the treatment process.
  • Evaluate the effectiveness of individualized treatment plans on patients' progress in resolving identified problems and moving towards defined objectives.
  • Coordinate patient care with members of the Housing Team and other Clinic departments.
  • Participate in all appropriate team meetings and required training sessions.
  • Maintain active, unrestricted ARNP license.
  • Be comfortable working with individuals living on the street or in the woods as well as in a clinic setting.
  • All other duties as assigned.
