Part time 20-30 hrs/wk
The Prescriber offers psychiatric assessment and medication management within the outpatient Youth and/or Adult Psychiatry setting in the Health Center Program. This position is responsible for individual services to patients assigned by Crossroads Health. This licensed provider will engage as a member of a collaborative team. With a Trauma Informed approach, this position will be a representative for the organization and how we offer integrated services for recovery and mental health.

Additionally, the Prescriber will:
Provide psychiatric assessments, medication evaluation and medication management.
Regularly assess consumers & prescribe medications for clients via office and home visits
Educate consumers & their families regarding medications/symptoms/illness/side effects. .
Participate in ongoing assessment and therapeutic engagement. Complete, as directed, elements of the comprehensive assessment for clients within 60 days of admission.

Requirements

• Master's Degree (minimum)
• Licensed in the State of Ohio as a Physician Assistant, Psychiatric Nurse Practitioner, or Physician
• Controlled Substance Registration Certificate
• Medicare Provider Number
• Professional Liability Insurance.
• Standard Care Arrangement (annually) or Supervision Agreement (whichever applicable)
• BLS/First Aid (employer provided)
• Must have valid driver's license
