Our care teams are composed of the following: Physicians-The patient’s Attending Physician continues to provide oversight of care and Hospice Physicians consult with hospice staff and the attending physician. Nurses (RN or LPN) Social Workers Certified Nursing Assistants (Home Health Aides) Spiritual Care (Chaplains) Pharmacists Trained Volunteers- offer companionship for patients and respite for families Job Summary: Plans, develops, and evaluates the Hospice Grief and Bereavement Program in accordance with the Hospice Conditions of Participation. Responsible for developing Hospice bereavement plans of care and for coordinating and overseeing the implementation of those plans. Responsible for all training, education and development of educational materials related to grief and bereavement in the Hospice program. Collaborate with the Hospice Volunteer Coordinators to train, supervise, evaluate and support the Hospice bereavement volunteers. Is an active member of the Hospice Leadership Team and the Hospice Interdisciplinary Team.
