Glasgow State Nursing Facility (GSNF) may be the place for you. We are a state-of-the-art 100-bed long-term care nursing facility for individuals with mental illness or intellectual disabilities. We are seeking a BH/DID Program Supervise for our Social Service Department to join our team in caring for and improving the daily lives of our residents. The position you are applying for has access to or use of federal tax information (FTI) or meets the statutory definition of front-line staff. Therefore, pursuant to IRS Publication 1075 and 900 KAR 1:009 or KRS 194A.062 and 900 KAR 1:050, applicants for this position in the Cabinet for Health and Family Services (CHFS) shall submit to a fingerprint-based criminal background check by the Department of Kentucky State Police and the Federal Bureau of Investigation. Requirements

  • Graduate of a college or university with a bachelor's degree.
  • Three years of professional experience in social work in a behavioral health/developmental and intellectual disability treatment setting or counseling in a behavioral health/developmental and intellectual disability treatment setting.
  • Excellent verbal and written communication skills.
  • Interpersonal skills with the ability to manage sensitive and confidential situations with professionalism.
  • Attention to detail and excellent organizational skills.
  • Proficient with Microsoft Office and computer skills.

Responsibilities

  • Manages the admissions process including scheduling and completing appropriate paperwork and distributing to applicable personnel. Enters potential admissions into KLOCS and tracks PASSAR (Pre-Admission Screening and Resident Review). Coordinates referrals to other facilities and completes appropriate discharge paperwork per policy. Ensures all regulatory requirements for admissions and discharge are met.
  • Completes individualized psychosocial assessments and plans of care for new residents and quarterly for all other residents. Participates in interdisciplinary committee meetings as needed and serves as an advocate for the residents. Ensures applicable portions of MDS are completed timely. Enters data in computer systems assuring compliance with facility, state, and federal mandated regulation, policies, and procedures.
  • Provides therapeutic counseling and case coordination for individuals with mental illness or developmental disabilities in a long-term care facility.
  • Works with the court and legal system on transfers and guardianship. Maintains positive interaction with resident representatives and assists with resident representative meetings. Assists with advanced directives and applicable forms.
