Join the NVFS Family: We are looking to hire a bilingual Spanish-speaking Case Support Case Manager to join the Youth Initiatives team! The Bilingual Case Support Case Manager in the Youth Initiatives Programs provides community-based clinical case management services to youth receiving funding through the Children’s Services Act (CSA), that provides disciplinary planning to help meet the needs of children with significant emotional or behavioral challenges and their families in Virginia. The Case Manager will focus specifically on youth and families who are Spanish-speaking and/or immigrant families and eligible for support through CSA. The Case Manager will serve as the Lead Case Manager for the youth and their family. Requirements

  • Master’s degree in Social Work, Marriage and Family Therapy, Counseling or equivalent
  • Oral and written fluency in English; Spanish language fluency required
  • Must have at least two (2) years of related direct service experience providing: individual and/or group services to youth and families, case management, and working with youth with behavioral health care needs
  • Digital and technical proficiency including working knowledge of Microsoft Office Suite
  • Must be able to demonstrate and live the organizational values of Communication, Integrity, Respect, Collaboration, and Accountability.
  • Successful completion of background checks upon hire
  • Must clear Motor Vehicle Record check, have valid S. driver’s license, and have own transportation in order to transport clients
  • Must have reliable transportation to visit client homes and other designated community locations
  • May be called upon to work beyond normal working hours

Nice To Haves

  • Experience providing services to Spanish speaking immigrants and recently reunified families and youth strongly preferred

Responsibilities

  • Provide clinical case management services for a culturally diverse population of youth and their families, which includes the following duties: Administration of the CANS
  • Assisting individuals and their families with access to services and supports
  • Collection and summary of relevant history and assessment data and representation of such information to the Family Assessment Planning Team
  • Development of the Individualized Family Service Plan/Mutual Assessment Plan for community- based services to include natural supports and transition planning as appropriate
  • Liaison between the family and service providers
  • Attendance and provision of behavioral health care expertise at any necessary Family Resource Meetings and Family Planning Meetings
  • Quality assurance of service provision by monitoring direct service providers, and progress towards goals by maintaining regular contact with clients and team members
  • Documentation of activities in agency electronic health care record in compliance with State Performance Contract, team practice and contract agreements
  • Completion of the responsibilities assigned to CSA case managers and TBP participants in local CSA policy
  • Collaborate with other providers to ensure holistic service provision
  • Ensure that service delivery is performed in accordance with the NVFS mission, agency policies and procedures, case management model, and project requirements
  • Perform other indirect service activities that may include: program development, community outreach, or Agency committee work
  • Prepare statistical and narrative reports as required
  • Participate in regular individual (administrative and clinical) and group supervisions as directed by supervisor
  • Assure confidentiality policies and procedures are followed to protect the rights of clients, that reports to authorities occur in the mandated time frames and that critical incident reviews related to these procedures are completed and filed
  • Maintain client files in accordance with program/licensing requirements and NVFS guidelines
  • Participate effectively and positively in required Program and Agency meetings and trainings
