About The Position

ATW Health Solutions is a mission-driven organization advancing equity, improving health outcomes, and strengthening systems through innovative, evidence-based strategies. We collaborate with healthcare providers, community organizations, and government agencies to address disparities and ensure the voices of underserved communities are heard and acted upon. We are looking for a compassionate, community-oriented individual to join our team as a Community Health Navigator. In this role, you will help individuals and families in Cook County connect to essential healthcare services, including support with Medicaid enrollment and renewals. You do not need prior experience with Medicaid redetermination — we provide comprehensive training and ongoing support. What matters most is your ability to connect with people, communicate clearly, and care about improving access to healthcare in your community. This contractor role is ideal for someone who: Is bilingual in Spanish and English Enjoys working directly with community members Is dependable and timely Is comfortable engaging with people in public and community settings Is passionate about helping others access healthcare resources We Provide Training on Medicaid enrollment and redetermination Ongoing support from a collaborative team An opportunity to make a meaningful difference in your community If you are bilingual, passionate about helping others, and interested in growing your skills in community health, we encourage you to apply, even if you don’t meet every qualification listed.

Requirements

  • Fluency in Spanish and English (strongly preferred)
  • Friendly, approachable, and culturally responsive communication skills
  • Ability to work independently in community settings
  • Commitment to health equity and serving diverse populations

Nice To Haves

  • Experience in community outreach, customer service, healthcare, or social services is helpful — but not required

Responsibilities

  • Support community members at local events, health fairs, and partner sites
  • Provide one-on-one guidance in a welcoming and respectful manner
  • Help individuals understand their healthcare coverage options
  • Connect residents to Cook County Health services and resources
  • Participate in training and team meetings to build your knowledge and skills
