About The Position

The Outreach Coordinator (OC) is responsible for coordinating and facilitating clinical and nonclinical community events, including mobile clinics, school-based health services, and outreach activities in Chelan and Douglas counties. This position serves as a vital bridge between Columbia Valley Community Health (CVCH), schools, and the broader community, ensuring seamless integration of health services. The OC supports patient access, promotes positive health behaviors, and fosters strong partnerships with community organizations and school personnel. This role requires excellent coordination, customer service, and organizational skills to improve health outcomes and facilitate community engagement.

Requirements

  • High School diploma or GED required.
  • Current Healthcare Provider CPR certification required (training provided if needed).
  • Eligible for Medical Assistant-Registered Licensure (CVCH will provide required training).
  • Seven hours of AIDS education and training required (provided by CVCH).
  • One year of experience in a healthcare setting, social services, or outreach coordination preferred.
  • Experience working with children, families, and diverse populations preferred.
  • Bilingual in English/Spanish Required.
  • Fluent written and verbal communication in English/Spanish.
  • Knowledge of computer applications and equipment related to work.
  • Proficient keyboarding skills and must have strong experience with Microsoft Office software.

Nice To Haves

  • Associate’s degree or higher in Public Health, Sociology, Social Work, or a related field preferred.
  • Community Health Worker Certification preferred.
  • State Certified Translator preferred.

Responsibilities

  • Outreach and Community Health Coordination Coordinates and schedules clinical and nonclinical outreach services and events.
  • Organizes and oversees mobile health services, school-based health clinics, and other community events.
  • Develops and maintains relationships with community partners, schools, and CVCH staff to enhance outreach efforts.
  • Assists in the onboarding, orientation, and training of staff and volunteers involved in outreach activities.
  • Maintains scheduling for outreach events, forecasting three months in advance to ensure adequate staffing.
  • Ensures compliance with department policies, safety standards, and infection control protocols.
  • Serves as a primary point of contact for school-based health services, ensuring seamless coordination of care between CVCH and school districts.
  • Identifies and prioritizes students in need of health services, advocating for students facing barriers such as transportation challenges.
  • Works closely with school administrators, nurses, counselors, and staff to integrate health services within the school environment.
  • Organizes and supports on-site health services, including physical exams, immunizations, screenings, and mental health services.
  • Tracks and reports student health data, monitoring Key Performance Indicators (KPIs) and program outcomes.
  • Leads student development activities such as health fairs and workshops, promoting wellness education.
  • Engages with parents and families, providing information and resources to support student health and academic success.
  • Registers patients, collects and updates patient demographics, and schedules appointments following established protocols.
  • Assists with patient screenings, including vital signs, finger-stick blood tests, and CLIA-waived lab procedures.
  • Observes and reports patient symptoms and maintains accurate medical and immunization records.
  • Ensures universal health screenings are conducted, addressing unmet health needs and connecting students to appropriate care.
  • Adheres to standard precautions and disposes of biohazardous materials safely.
  • Conducts reporting as required by department management, ensuring data accuracy and compliance.
  • Assists in department workflow improvements and strategic planning efforts.
  • Manages ordering and inventory of outreach materials and medical supplies.
  • Documents all student interactions, interventions, and follow-ups in school health records and case logs.
  • Participates in staff performance evaluations and provides hiring feedback as needed.
  • Performs additional duties as assigned by the department manager.
