Kinship Navigators support and serve kinship families utilizing a strength-based approach to provide ongoing support and case management services. Kinship Navigators participate in the development of a Navigation Plan setting goals that guide the course of services. Kinship Navigators make appropriate referrals to ancillary services and community resources, and track and monitor all services in the database. Navigators make referrals to ACH programs and other community agencies based on assessment of family needs with an emphasis on meeting the needs of Kinship Caregivers and the children/youth they care for. Navigators collect and distribute client information to other ACH programs as needed for the purpose of service coordination. Navigators participate in community outreach and collaboration to increase awareness of KNP services. Kinship Navigators offers information on community resources to ACH program staff available to Kinship Caregivers, youth, and families. The job requires sensitivity to the service population’s cultural and socioeconomic characteristics.
