About The Position

Homebridge believes that quality care comes from skilled and supported caregivers. As California’s largest employer-trainer of caregivers and the State’s largest IHSS agency provider, Homebridge operates multiple programs that benefit caregivers and their clients across the State. In San Francisco, Homebridge provides nearly 1/2 million hours of in-home care to more than 1,000 complexly diagnosed IHSS Medi-Cal beneficiaries each year, with a staff of more than 300 caregivers. Beginning in 2023, Homebridge is also a provider of Enhanced Care Management (ECM), a benefit under the California Advancing and Innovating Medi-Cal (CalAIM) program. ECM is provided as a benefit from Medi-Cal Managed Care Plans (MCPs) for members who fall into specific populations of focus that Homebridge serves, including: Adults living in the community who are at risk of Long Term Care (LTC) institutionalization Adults experiencing, or at risk of, homelessness Adults who experience frequent, avoidable utilization of emergency departments and/or hospitalizations Adults with Serious Mental Illness and/or Substance Use Disorder (SMI/SUD) In 2026, the CalAIM portfolio is expanding to provide two additional CalAIM benefits: Community Health Workers (CHWs) and Community Supports (CalAIM-CS). Initial CalAIM-CS focus is on the following Community Supports: Housing Transition Navigation Services (HTNS) Housing Tenancy and Sustaining Services (HTSS) Day Habilitative Programs Personal Care and Homemaker Services Respite Care We are passionate about creating impact on the lives of those we serve. The Cal AIMS Program Manager is a member of Community Care leadership along with other program managers and reports directly to the Senior Manager of Cal AIMS Services. The Cal AIMs Program Manager supervises staffing and supports operations of Cal AIM programs such as Enhanced Care Management, Community Supports, and Community Health Workers. The ECM Manager ensures that day to day program operations meet the expectations of both clients and MediCal Health Plans who partner with HomeBridge to offer services to their members.

  • BACHELOR’S DEGREE OR EQUIVALENT EXPERIENCE WITH THREE OR MORE YEARS SUPERVISING OR MANAGING STAFF, OR MASTER’S DEGREE OR EQUIVALENT EXPERIENCE WITH TWO YEARS SUPERVISORY EXPERIENCE REQUIRED.

Responsibilities

  • Is a member of the management team providing leadership to staff and participates in decision making processes
  • Ensures high quality delivery of services by staff providing Cal AIMs related activities to medically and behaviorally complex clients.
  • Ensures program operations meet contractual and regulatory requirements, and relays any potential risk for non-compliance to the Cal AIMs Senior Manager
  • Gathers and uses data regularly to monitor and manage self, others and programs’ performance; identifies issues and recommends improvements
  • Directs supervision of care teams; managing Care Supervisors directly, for teams involved in non-partnership programs. Supervision is full cycle from hiring, training, regular one on ones, and performance management
  • Facilitates regular meetings and group sessions to share information, solve problems and enable peer support
  • Ensures that teams employ best practices to mitigate client and staff safety hazards, including regular training and retraining as needed
  • Directs Care Supervisors in the handling of on-the-job injury, leaves of absence and employee performance concerns, referring to Human Resources as necessary
  • Problem solves, mediates, and facilitates communication between all team members to foster a teamwork approach among administrative team members, HCPs, and clients
  • Demonstrates commitment to client care and quickly works to solve issues involving clients.
  • Comprehensively screens referred client intakes to evaluate services required; assigns Care Supervisors, ensures that care is initiated within contractual time frames; monitors quality of care provided
  • Works closely with the Program Director, Cal AIMs Senior Manager and other departmental Managers to develop and implement program improvements
  • Works collegially with partners including Medical Managed Care Plans, enabling collaboration regarding client’s needs for services, including changes in visits, community resources, and access to care.
  • Ensures that client services are provided and in compliance with contractual requirements and goals as authorized by the Managed Care Plan and program requirements
  • Completes reports and makes recommendations on improvements using information from program operations.
  • Establishes and implements Quality Assurance mechanisms to maintain high standards of care and programming, including regular chart reviews and peer review
  • Tracks data related to contract compliance, client complaints, etc.
  • Acts as mandated reporter reporting known or suspected abuse of dependent adults or elders
  • Interacts with other agencies and service providers in the community to share information about services and resources available to Cal AIMs recipients
  • Builds new relationships and maintains existing relationships with other community-based organizations, IHSS, community resources and city/county agencies supporting our clients
  • Provides programmatic support as needed after hours and for specialized training
