About The Position

Lutheran Services Florida (LSF) envisions a world where children are safe, families are strong, and communities are vibrant. LSF is seeking a talented Care Coordination Specialist who wants to make an impact in the lives of others. Purpose and Impact: The role of the Care Coordination Specialist is to ensure integration of any service systems related to Substance Abuse and Mental Health in the Northeast Florida Region. This position is used to establish collaborative associations that positively effect change in Substance Abuse and Mental Health service systems of this region. This position effectively implements the care coordination program while fostering effective communication with the community and service providers, and promoting the mission, vision and core values of LSFHS.

Requirements

  • Bachelor’s degree from an accredited college or university with a major in social service or related fields required.
  • A minimum of 4 years of social service experience required.
  • Demonstrated ability to communicate effectively, constructively and supportively with staff, providers and community stakeholders while also maintaining high standards of excellence.
  • Must show ability to work independently and be able to manage time efficiently.
  • Must be able to use discretion and judgment to implement management policies when interacting with community members.
  • Must demonstrate sensitivity to our service population’s cultural and socioeconomic characteristics and needs.
  • Reports to the Director of Program Operations
  • Team player with providers, co-workers and office staff.
  • Accurate, complete and timely submission of required reports and proposals.
  • Adherence to policies and management practices.

Nice To Haves

  • Master’s preferred.
  • License (LMHC/LCSW) preferred.

Responsibilities

  • Build partnerships/collaborations with local agencies, organizations, schools, clinics, hospitals, churches, etc.
  • Identify needs of the targeted communities and how the various systems of care work together.
  • Implement, and evaluate a care coordination plan to enhance care coordination services at the individual consumer level.
  • Ensure that assessment, linkage to treatment, and treatment maintenance and follow-up services are in place to monitor consumers enrolled in the care coordination program.
  • Identify high utilizers, complex special need consumers and other target priority populations at the system level
  • Provide relevant data informed surveillance monitoring to identify individuals in need of care coordination.
  • Contact providers and conduct case staffing regarding consumer(s)’ needs, and wrap-around services.
  • Develop and implement multiagency release of information to increase information sharing across the system of care.
  • Provide technical assistance and resource allocation to providers.
  • Monitor outliers, target priority populations, and need for additional/different services.
  • Ensure network adequacy and manage resources.
  • Assess and address quality of care issues based on established standards of care.
  • Develop cross system partnerships and agreements for information sharing and coordination.
  • Assist in eliminating barriers at the systems level to address regional and local Behavioral Health resource issues.
  • Data portals for all available services – MEs to connect with each other statewide and in future, other funders. Managing Entity will monitor system of care and work with providers and other stakeholders.
  • Identify and contract with provider(s) to provide service coordination with clear expectation and ongoing monitoring.
  • Manage & monitor care coordination funds for purchases of needed services/supports.
  • Assist in enhancing ways to monitor care coordination activities.
  • Assist community agencies in leveraging other available resources toward a better overall system of services.
  • Solicit and receive feedback from providers and community stakeholders on service provision, community needs, and system of care development and reporting back to Managing Entity staff and Director of Program Operations
  • Other tasks as assigned
