Requirements

  • High School Diploma or GED - Required
  • 2 years Direct patient care, outpatient, ambulatory care, specialty care, or community healthcare experience - Required
  • Basic Life Support (BLS) CPR Card - Required

Nice To Haves

  • Associate's Degree Business, Health Administration, or related field - Preferred
  • Strong writing and computer skills, including MS Word, Excel, EMR (electronic medical record), medical terminology. - Preferred
  • MA, EMT, or equivalent military experience; Medical Assistant Certificate - Preferred

Responsibilities

  • Maintains and manages their caseloads working independently with a minimum of direction, anticipate and organize work flow, work with a high volume caseload, and deal with rapidly changing priorities. Works collaboratively with clinical and research staff, social service organizations and community resources, as needed, to meet each patient’s needs. Maintains a working knowledge of and can effectively communicate patient associated costs of research and non-research related costs, applicable laws and regulations, government (Medicare, Medicaid, SSI, and SSDI), and insurance benefits. Ensures accurate and complete documentation is recorded for every patient encounter. Utilizes active listening, health literacy principles, including teach back to ensure effective communication with patients and caregivers. Participates in efforts to improve processes and procedures which could include participating in taskforces, committees, and special projects.
  • In conjunction with clinical and research practitioners such as RNs, doctors, nurse practitioners, social workers, and clinical research staff coordinates and reviews patients’ progress and ability to manage health care issues. Performs outbound calling, medical records requests, appointment scheduling, and completing referrals requests as necessary based on patient need. Communicates professionally using all modalities (email, technology, written, and verbal) with clinical team, patients, and care givers in a way that is clear and concise.
  • Assists patients with their medication reconciliation, self-management support, follow-up care, supportive care, community resources, and long-term planning needs. Maintains communication with clinical and research staff regarding status of patient, reporting any issues or concerns from the patient or patients’ caretakers in routine patient meetings and in clinic with physicians/NPs. Follow-up on outcome of patient medical care instructions and reports to appropriate clinical staff. May assist with the preparation of annual reports for Standard of Care and Clinical Research patient data. Assist leadership with conduct of monthly quality check of data and metrics.
  • Ensures safe handoff for care transitions into clinical care and/or in clinical trials from Care Coordinators to other healthcare or research staff. Coordinates follow-up care with PCP or/ Specialists regarding outpatient follow-up appointment and plan of care. Coordinates care with internal and external providers and healthcare team members involved in the care. Builds and maintains working relationships with patients and their care givers and other health care partners.
