Care Coordinator, RN
About The Position
The Care Coordinator, Registered Nurse is assigned to a specific clinical care area, facilitating the plan for outpatient care by using a collaborative interdisciplinary approach. The Care Coordinator will focus on anticipation, integration and coordination of clinical care necessary to meet the needs of patients and to optimize clinical and resource outcomes. The Care Coordinator collaborates with the interdisciplinary team in assessing patient needs and developing the plan of care. The Care Coordinator acts as an advocate and resource for the patient, family, providers and department relative to compliance with federal, state and third party utilization management regulatory requirements.
