Care Coordinator
About The Position
As a Care Coordinator, you’ll play a vital role in creating meaningful moments for the patients and families we serve. You’ll be the compassionate guide who helps them navigate their care journey, ensuring they feel supported, informed, and truly seen at every step. Requirements
- Minimum Required: Associate’s Degree in a Human Service-related field
- May accept 5+ years of related care coordination experience in lieu of an Associate's Degree.
Nice To Haves
- Preferred: Bachelor’s Degree in Science or Arts in Human Services related field
Responsibilities
- Outreaches and engages with UHS System patients for care coordination/care management.
- Assists in care coordination efforts as a member of the care team.
- Documents efforts in electronic medical record to include but not limited to NextGen, and Allscripts.
- Develops and implements care plans as a member of the care team.
- Attends care conferences as necessary.
- Also, may organize and/or lead care conferences as necessary.
