Flexible Work Schedules Remitted Tuition Professional Development Competitive Pay and Work Life Benefits Professional Supervision and Coaching Recognition and Appreciation Programs Opportunities for Growth and Promotion The Care Manager plays a vital role in Adult Health Home Case Management by actively engaging clients through in-person contact, assessing their needs, and providing comprehensive care coordination. They identify eligible clients, educate providers on eligibility criteria, and collaborate with the lead health home to determine eligibility. The Care Manager develops patient-centered plans of care, prepares crisis intervention plans, and ensures the delivery of all necessary services by coordinating with service providers and health plans. They advocate for services, assist with scheduling medical appointments, and provide support for transitional care following admissions and discharges. Additionally, the Care Manager offers patient and family support, connects them to community resources, and ensures timely and thorough documentation as required by the Department of Health. This Position Is Full Time At 35 Hours A Week. Pay For This Position Is $23.50 Per Hour

Requirements

  • Bachelor's degree in Nursing or Human Services related field
  • Minimum of 3 years of experience in care coordination or equivalent combination of education and experience
  • Ability to work with people of diverse backgrounds
  • Thorough working knowledge of local and regional public and private resources
  • Familiarity with Social Security, Department of Social Service Adult Protective Unit, Income Assistance Unit, and other relevant agencies
  • Basic computer skills
  • Creative problem-solving skills
  • Negotiation skills
  • Ability to navigate and utilize multiple Electronic Medical Records (EMRs)
  • Valid N.Y.S. Driver's License
  • N.Y.S. Child Abuse Registry clearance
  • Mobility sufficient to drive to and participate in meetings at client's homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals
