About The Position

Care Manager Mentor, Full-Time-Salaried Opportunity Community Care is hiring a Care Manager Mentor to join our team. Community Care’s mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This full-time opportunity offers a weekday schedule. It is based in Waukesha with telework currently being the norm. About Community Care Community Care is a local nonprofit with more than 40 years of experience helping older adults and adults with disabilities live as independently as possible within the community. We coordinate and deliver a full range of supportive services that help more than 13,000 Wisconsin residents live safely, confidently, and with dignity. We are Wisconsin-based with local offices supporting each of the 23 counties we serve Our dedicated team works with members to develop care plans to meet their health and social needs Together with our community partners, we’re committed to providing compassionate, personalized care that empowers members to live a healthy and independent lifestyle.

Requirements

  • Education: Bachelor’s Degree in Social Work, Psychology or Human Services and 18 months experience with older adults or persons with physical and/or intellectual disabilities in a social service capacity OR a Masters’ Degree in Social Work, Psychology or Human Services.
  • Certification: Long-term care functional screener certification is required
  • Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Responsibilities

  • Provides on-the-job training, orientation and feedback to new Care Managers related to their assigned caseloads and job tasks.
  • Serves as an on-going resource to Care Managers.
  • Regularly communicates with the direct supervisor information about the new employees’ progress, needs for improvement and/or educational training.
  • Participates in efforts to enhance work experiences.
  • Coaches staff to achieve the highest level of evidence-based practice.
  • Leads and supports to enhance employees’ work experiences and retain talent.
  • Responsible for all job specific responsibilities listed in the Care Manager job description for Family Care or Family Care Partnership, as appropriate.
